J&B Medical is providing you access to health information via this web-based Provider Portal (the "Provider Portal") under the following terms and conditions. You are required to review and accept these Terms and Conditions in order to access the Provider Portal.

I understand that I am requesting access to protected health information (PHI) in the Provider Portal. I certify that I am over the age of 18 and that I am a medical professional. I will access information only on those patients with whom I have a current treatment relationship, or who have scheduled an appointment with me and/or my organization.
I will access the Provider Portal only for the purpose of treatment of my patients. I understand that my use of the Provider Portal will be audited for appropriate use.
I shall maintain the confidentiality and privacy of all information obtained from the Provider Portal and shall comply in all respects with HIPAA, and all other laws governing the use and disclosure of the information. I understand that J&B Medical accepts no responsibility for and disclaims any and all liability or consequential damages arising from a breach of health information confidentiality due to my failure to fulfill my obligations under these Terms and Conditions.
I understand that my username and password are unique codes that identify me in the Provider Portal computer system and allow me to access PHI. I am responsible for: (1) keeping my username and password confidential; (2) changing my password on a regular basis; and (3) immediately notifying J&B Medical if I believe my username or password may have been compromised. I will notify J&B Medical immediately of any event (change in employment status, position, title, eligibility, etc.) that may affect my legal right or authority to access PHI in the Provider Portal.
My continued use of this Provider Portal indicates my agreement to abide by these Terms and Conditions, as may be amended from time to time.
All content included in the Provider Portal, including, but not limited to, text, photographs, graphics, button icons, images, artwork, names, logos, trademarks, service marks and data (the "Content"), in any form including the compilation thereof, are owned and controlled by J&B Medical or are owned and controlled by third parties and licensed to J&B Medical. Except as set forth below, direct or indirect reproduction of the Content, in whole or in part, by any means, is prohibited without the express written consent of J&B Medical.
J&B Medical grants you a limited license for your own use of the Provider Portal and the associated services in accordance with these Terms and Conditions. This license expressly excludes, without limitation, any reproduction, duplication, sale, resale, or other commercial use of the Provider Portal and the associated services, making any derivative of the Provider Portal or the associated services.
The Provider Portal is not designed to make diagnoses. Only an authorized provider can properly diagnose and treat an illness or injury.
I understand that all health-related information provided through the Provider Portal is intended to be for general use and should not be used as a substitute for the expertise of a healthcare professional. J&B Medical is not responsible for how I use the information obtained.
The Provider Portal may not be available to me all the time. Access is provided on an as-is, as-available basis. J&B Medical does not guarantee you will be able to access the Provider Portal at any particular time due to system failures, backup procedures, maintenance, or other causes beyond the control of J&B Medical. J&B Medical reserves the right to change the features of the Provider Portal and these Terms and Conditions at any time.
The undersigned party recognizes and consents to the following terms and conditions for access to the Provider Portal:
For purposes of this consent, "we", "us", or "our" means J&B Medical, Inc., and "you" and "your" means the person giving this consent.
Your consent to this Electronic Signature Agreement covers the transaction you are presently completing (e.g. submission of a deferment, forbearance, auto-debit request, etc.), which you will be prompted to sign electronically. You understand and agree that your electronic signature of the transaction you are presently completing shall be legally binding and such transaction shall be considered authorized by you. You understand that by checking the box and agreeing to sign electronically, your electronic signature has the same legal force and effect as a handwritten signature.
At any point in this process, you will be able to print and read the information that is presented to you using your browser print option. However, the document you print upon completion of the electronic signature process may not be a complete version of the document due to system limitations and differences of technology. If you would like to receive a true and exact copy of the signed document, you may contact us.
At any time prior to submitting your electronic signature, you may opt out of the electronic signature process and continue with a paper process. Simply exit this session prior to accepting this Electronic Signature Agreement. You may contact us to receive and continue with a paper copy of the transaction you are presently completing.
To sign electronically, you will be prompted to enter your password. This will be used to represent your name and date signed on the electronic document along with the words Electronically Signed. Clicking submit completes the electronic signature process.
In order to electronically sign the transaction you are presently completing, you will need the following minimum hardware and software requirements, which you acknowledge having by proceeding with this transaction and giving us your consent:
You acknowledge and agree that your consent to your electronic signature is being provided in connection with a transaction affecting interstate commerce that is subject to the federal Electronic Signatures in Global and National Commerce Act, and that you and we both intend that the Act apply to the fullest extent possible to validate our ability to conduct business with you by electronic means
Each provider who utilizes an electronic signature is required to agree to and follow J&B Medical’s Provider Portal Terms & Conditions and Electronic Signature Agreement. The agreement will be permanently retained in the provider’s J&B Medical file.
All electronic signature users will certify that they will not disclose their user identification and password or another device to anyone, which would permit someone to use their electronic signature.
Providers participating in the electronic signature process must agree to electronically sign all available prescriptions and other documents when they are made available electronically by J&B Medical. Manually signed prescriptions and other documents may not be accepted or processed as timely as those signed electronically.
Should a provider not remember their password, they can contact J&B Medical and after appropriate identification is provided, they will be assisted with their password assignment
